Managing Chronic Conditions Between GP Visits

If you have a chronic condition like diabetes, hypertension, asthma, or arthritis, you know that managing it is a daily commitment. With GP access becoming harder, self-management skills are more important than ever.
General Principles
1. Know Your Numbers
Whatever your condition, there are key numbers to track:
- Diabetes: Blood glucose levels, HbA1c
- Hypertension: Blood pressure readings
- Asthma: Peak flow readings
- Heart conditions: Pulse rate, weight
2. Take Medications as Prescribed
- Set reminders on your phone
- Use a pill organiser
- Don't stop medications without consulting your GP
- Order repeat prescriptions in advance
3. Recognise Warning Signs
Know when your condition is worsening and what to do:
- Diabetes: Signs of hypo/hyperglycaemia
- Asthma: When to use your reliever vs preventer inhaler
- Heart failure: Sudden weight gain, increased breathlessness
