Managing Chronic Conditions Between GP Visits

Living with a Long-Term Condition

If you have a chronic condition like diabetes, hypertension, asthma, or arthritis, you know that managing it is a daily commitment. With GP access becoming harder, self-management skills are more important than ever.

General Principles

1. Know Your Numbers

Whatever your condition, there are key numbers to track:

  • Diabetes: Blood glucose levels, HbA1c
  • Hypertension: Blood pressure readings
  • Asthma: Peak flow readings
  • Heart conditions: Pulse rate, weight

2. Take Medications as Prescribed

  • Set reminders on your phone
  • Use a pill organiser
  • Don't stop medications without consulting your GP
  • Order repeat prescriptions in advance

3. Recognise Warning Signs

Know when your condition is worsening and what to do:

  • Diabetes: Signs of hypo/hyperglycaemia
  • Asthma: When to use your reliever vs preventer inhaler
  • Heart failure: Sudden weight gain, increased breathlessness
